One of our professional care managers will be at the other end to learn more about your situation… so we can best begin to address your needs. In critical or emergency cases, services can begin immediately. Regardless of the situation, we will work with you to carefully plan out a course of action using a simple two-part evaluation process.
We treat all initial calls as professional consultations. You will speak with one of our care managers. He or she will listen to your situation and help you determine whether our services are right for you. In this consultation we will discuss the issues that most concern you and provide you with advice on how to help you with these issues.
If you decide to continue with us following the consultation, we will arrange for a free assessment. Often, that assessment can take place within as little as 24 hours wherever you are: home, hospital, assisted living facility or nursing home.
Step 2 - Free Assessment/Consultation by a Care Manager
One of our Care Managers will conduct a free, in-home comprehensive assessment of you or your loved one's physical and emotional health as well as their living situation. Issues covered include, or in as little as an hour in an emergency situation:
- Description of Services
- Daily routines
- Health and medication issues
- Emergency response
- Nutritional issues
- Home cleaning issues
- Transportation needs
- Costs of services
- Privacy and confidentiality
- Client’s Rights
This assessment provides you and your family with actionable solutions to your caregiving problems by formulating a plan of care that fulfills your unique needs. It also allows us to appropriately match the right caregiver to meet your needs. You will always have the right to accept or refuse any caregiver we propose.
You will then sign a service agreement that details costs, services, rights, emergency contact information and other important information you need to have at your finger tips. This is not a contract, but our agreement to provide only the services you request.
Once you have your personal plan of care in place, Premier Caring Hands will put our wide range of services into action. Caregivers (or The Caregiver) will document all services rendered during any shift worked. Care Managers will summarize services on a monthly basis in a report that many clients find useful to share with their primary care physicians or specialists. These reports are available on line with a personal password and pin for confidentiality and ease of information exchange.
All of our caregivers are employees of Premier Caring Hands (Insured and Bonded) and submit to extensive background checks and training.
